According to the regulations we have been given, inmates need to complete State Form #48405 "Request for Personal Preference Diet".

The form can be obtained through a prison counselor. It must be filled out, signed and, according to the regulations, must state "moral or religious reasons" for the request.

Once the form is filed, a Chaplain will schedule a meeting with the inmate to discuss the request.

If the request is approved, the Chaplain will give the inmate a  "Personal Preference Diet Card - State Form #48407.  The Chaplain's office will also notify the Food Service Department of the inmate's approved diet..

Please note that inmates must present the approved card at each meal in order to receive the special meal.

Indiana Ombudsman Program

Indiana DOC has an interesting program that allows inmates to directly contact an ombudsman with complaints.

According to their description: "The Indiana Ombudsman Bureau was created in 2003. The Ombudsman Bureau receives, investigates and attempts to resolve Offender complaints regarding the policies and procedures of the Department of Correction. The Bureau operates independently of the Department of Corrections and is housed in the Department of Administration.... Indiana Ombudsman complaint forms are available in the law library of each respective facility."
