We spoke with staff at the North Carolina Department of Corrections and were told that inmates do not need to file any forms or make a special request for vegetarian diet. There is a "common fare" diet that includes a "meat substitute" - you just have to ask for it.

We have been told by those in North Carolina SHUs (higher security Special Housing Units) that they need to file a "Religious Diet Form" and that vegetarian meals are only allowed to people of certain religions. We have not yet been able to confirm this or to get a copy of the form that is required.
The North Carolina Department of Corrections website does not make available special forms for requesting vegetarian diets - see the middle panel which explains that inmates do not have to file special requests.
